I had a guest who upon leaving left exterior door wide open and put the key in lock box, but left the lock box open. Luckily I was home when the guest left, but what if I wasn't, the door would of stayed open for hours.
I've already reached out to guest and he apologized
Other than this they were good guests.
I haven't reviewed them yet, I don't know what to write.

It would be fair I think in the review to say that this guest was not careful enough about house security. You might even want to specifically state that the house door was left wide open when the guest departed.
Before deciding what to say in the review, I suggest contacting the guest, mentioning that you found the door left wide open and asking the guest what happened and why that was left like that . The way that the guest responds will help you know what to say in the review.,

Hey there Sadie 34, this happens to me a lot even though I walk them through the house and show them how to shut and lock the doors. It is partly the fault of my door, as people think the back door, being steel, should just shut itself. They do it to my front door too, a regular wooden one, about every two months. On that one, I have a sign that lovingly implores people to shut it. On the back door, which I find open once a week, I have a constellation of signs arranged all over the door that say " shut the door, please" "shut the damn door" "shut the fucking door!" I can do that because it fits in with the personality of my house in the hood. Since I added the f- word, door negligence is down 75%.

It happens to me too, luckily it's a door less visible for passers and neighbors do it whilst home. So most passers suppose it's just open for air. But I do a door routine with everyone, which reduced it considerably.
